Referral procedure

How to arrange an initial consultation

You can refer yourself directly to Hampden House for therapy or you may be referred via your GP or consultant*.

We offer you the opportunity to fill in a confidential questionnaire and it return to us. This allows you to tell us a little about the difficulties you are experiencing and what you would like to get from therapy. Having this initial information about you helps us to understand what might be most helpful to you and to respond to your request as quickly as possible.  While we encourage you to use the questionnaire, the offering of an initial meeting will not be affected should you choose not to do so. You will usually be offered an appointment with a therapist within two weeks.

Many of our therapists are registered with the main health insurance companies such as BUPA, AXA, Aviva etc. If you are in doubt please contact us.

*If you are making a claim on medical insurance, then you may have to be referred by a GP or consultant.

How to obtain the questionnaire

The questionnaire is available to download from this website, in which case you can complete it online and print it off to be completed by hand.  Alternatively we are happy to send you a copy of the questionnaire by post. Just request one from the Practice Administrator on 0161 445 2099 or There is further information about how confidentiality is assured with your questionnaire if you follow the link marked Confidentiality.
